What is a photography intake form?

Short answer

A questionnaire the client fills out at booking to give the photographer the details needed to plan the shoot.

Full definition

An intake form (also called a wedding questionnaire or session questionnaire) collects the answers a photographer needs before showing up — wedding date, venue, family group shot list, color palette, key people to capture, dietary restrictions for catered shoots, etc. Modern intake forms are embedded in the client portal so answers stay tied to the project. 12img intake forms are customizable per session type.
